Every permit carries a named applicant, approver, work window, location, and work type before high-risk work is allowed to begin.
The facility owner or safety approver can require serial sign-off, independent approval, and pre-work evidence before the job moves forward.
The crew completes pre-work checks, post-work close-out, and delegated sign-off on the same operating record.
01 · Applicant and approver
Both sides are named before the work begins.
A work order says what needs doing. A permit to work says who is asking to perform a risky activity, where it will happen, when it is valid, what hazards are declared, and who is responsible for allowing it on the facility.
Infodeck keeps that separation explicit. The applicant can be an in-house supervisor or a contractor crew lead. The approver can be the facility owner, compliance lead, or safety officer. Each signature records the person, role, time, and decision.

Different risky work needs different controls.
Every permit snapshots the rule it was issued under.
Hot work is not the same as excavation. Electrical isolation is not the same as working at height. Compliance defines a template for each permit type: required approvers, validity window, pre-work checklist, post-work checklist, hazards, PPE, and who can apply.

Your own work type
Every permit issued from the template carries a snapshot of those rules at the moment it was created. If compliance tightens the template next quarter, permits already in flight keep the standard they started under. No retroactive rule change.
03 · Pre-work checks
Approval is not the same as ready to start.
The site still has to be made safe.
Once the permit is approved, the crew still has to prove the work area is ready. The template names the pre-work checklist: hazards reviewed, PPE verified, site conditions photographed, isolations confirmed, and any work-type controls completed. Each response stamps onto the permit record, named to the person who signed. If the work order requires that permit, Infodeck will not let active work proceed until the authorization and required checks are present.

04 · Delegated checklist
Send the check to the crew.
No account required.
When the permit allows delegation, the supervisor generates a secure link for a named email. The link lands in the recipient's inbox with a 48-hour expiry. They open it on whatever device they have, complete the checklist, and submit. No login, no account, no new software for the crew.
The delegate cannot be the person who issued the delegation. Self-approval is refused by the engine. Whoever signs the check is not the same person who asked them to sign.

05 · Close-out
Risky work needs a signed finish.
When the work is done, someone attaches the post-work checklist response. The permit moves to post-work pending. A closing sign-off step, usually compliance or the facility owner, confirms the site is safe to hand back. The permit lands at closed. Expired permits terminate without a sign-off. Cancelled permits close with a reason on the record.
Close-out is not a phone call or a chat message. The closed record carries the applicant, approver, crew, hazards declared, PPE verified, every approval signature, every checklist response, and the timestamps for each.

06 · Audit trail
The record answers the auditor.
Every state change on a permit writes an audit row. Permit created, permit approved, permit started, post-work submitted, permit closed, permit cancelled. Each row carries the actor, the timestamp, the field changes, and the permit id. The crew members are named. The hazards declared are on the record. The PPE required is on the record. The contractor company and the contact are on the record.
When the auditor asks which permit covered the electrical isolation on Block A on the night of 2026-04-17, the answer is on the record. Reconstruct the day from the permit. No spreadsheet cross-reference.
